Terms of agreement and waiver of liability – Outdoor classes

Outdoor classes: 

  • I agree to adhere to all safety protocols as listed below and I will at all times maintain them. I will not leave my child alone at any time and will not ask a teacher to watch my child at any time. 

Outdoor Class Protocols:

  • We are following and adhering to guidelines from e city of San Francisco and medical professionals.
  • In-person classes may be canceled or moved online at any time.
  • If a class is canceled due to inclement weather, teacher illness, pandemic surges or fire smoke, or any reason beyond our control the class will be made up over Zoom.
  • No makeups will be allowed in person.
  • All in-person classes are limited to 6 families.
  • Out of respect for the most vulnerable of our population, we request that no one over 65 join our outdoor classes (they are always welcome on Zoom!)
  • Each class will be its own pod – no makeups in other in-person classes and absolutely no mixing of pods!
  • 6-10 feet of physical distance is required at all times. 
  • Please quickly make sure to keep child next to you if they would like to move outside of your "bubble."
  • There will be marked spaces for families to sit during class, measured 10 feet apart.
  • If attending at Waller, enter through the white gate, safely park your stroller, and find a marked place to sit.
  • Please maintain distance at all times even while parking strollers, please wait as each person arrives.
  • You must take 100% responsibility for keeping your child with you at all times
  • Masks must be worn by all participating adults and children over 2. 
  • Hands must be sanitized upon entry.
  • Each family can bring a set of instruments/props for use not to be shared with others.
  • Sick children and adults MUST stay home and attend online. 
  • No socializing on-premises! You can socialize in another location, and we can introduce you to one another via email, but we apologize – NO SOCIALIZING with teachers or each other at the 1350 Waller location.
  • Arrive 15 minutes early and leave as soon as class is over.
  • Request if possible to please try and use the bathroom at home before you leave. If there is an emergency, there are bathrooms you can use. But there are employees/workers at Waller so for your sake and theirs, please try to go at home. 
  • Any makeups for missed in-person classes must be scheduled in one of our online classes
  • No refunds are issued after the start of a session for any reason, in circumstances beyond our control., including Covid surges, natural disasters and all unforeseen circumstances – all classes will be online if the in-person classes need to be canceled.
  • You agree to the liability release waiver form below before attending your in-person class.

 

COVID-19 ASSUMPTION OF THE RISKS AND WAIVER OF LIABILITY

 

As consideration for San Francisco Music Together® permitting me to take in-person Music Together classes, I agree to the following: 

 

ASSUMPTION OF THE RISKS:  I understand that naturally occurring disease processes (including, but not limited to, the novel coronavirus or COVID-19) may exist in the community, including at San Francisco Music Together, and present an inherent risk that cannot be eliminated. I acknowledge that, although San Francisco Music Together is taking reasonable measures to avoid contact, transmittal, and exposure of disease processes like COVID-19 between people in our classes (including mask enforcement for all adults and children over 2, and strict 6-10 ft social distancing), it is ultimately my responsibility to ensure that I and my child take appropriate actions to safeguard ourselves. I understand and agree that as a registered member of San Francisco Music Together, I am accepting and voluntarily assuming the risk that I or my child may be exposed and become ill as a result of COVID-19 and other diseases.   

 

WAIVER OF LIABILITY: I, on behalf of myself and my child hereby voluntarily release, forever discharge, and covenant not to sue San Francisco Music Together, and its owners, managers, members, employees, agents, and volunteers (“Released Parties”) for any claims that may arise out of or relate in any way to my or my child’s exposure to any communicable disease, including but not limited to the novel coronavirus or COVID-19. The claims hereby released include, but are not limited to, claims of negligence against any of the Released Parties. 

 

I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS CONTENTS. BY ENROLLING IN CLASSES AT SAN FRANCISCO MUSIC TOGETHER, I UNDERSTAND I HAVE GIVEN UP CERTAIN LEGAL RIGHTS AND THAT THIS IS A BINDING LEGAL DOCUMENT.
