I agree to adhere to all safety protocols as listed below and I will at all times maintain them. I will not leave my child alone at any time and will not ask a teacher to watch my child at any time.
Outdoor Class Protocols:
To keep everyone safe and to be in compliance with the requirements set forth by the guidelines of 1350 Waller, All Saints church, we ask for your cooperation with our procedures. ALL OF OUR ALL TEACHERS ARE REQUIRED TO BE FULLY VACCINATED AND BOOSTED.
Outdoor Class Protocols:
(UPDATED 2/7/23):
- All of our teachers are required to be fully vaccinated and boosted.
- A maximum of 2 adults per family (one or two adults can attend per family).
- Only enrolled children can attend.
- You or the caregiver attending must watch your child and stay with your child at all times and intervene immediately if any unsafe behaviors such as throwing, hitting, etc. occur.
- OUTDOORS: Masks are optional for all attendees
- INDOORS AT 1350 WALLER: Masks are required for adults only (not for children).
- When entering the Restrooms Indoors at 1350 Waller adults are required to wear masks.
- Outdoor enrollees at 1350 Waller: We may go indoors at Waller on rainy days. When indoors at 1350 Waller adults must wear masks. Wearing masks for children is optional indoors.
- Sick children and adults must stay home and schedule a makeup, whether you suspect Covid 19 or not.
- You are welcome to schedule a make-up via the make-up scheduler when you are ill within the semester.
- If you or anyone in your home is exposed to or gets Covid 19, even if asymptomatic, please quarantine for 10 days, please schedule make-ups here.
- If your child throws a tantrum, or is loud in a way that is distracting to class please take them outside until they have calmed down.
- Only bring soft toys and small handheld instruments for the safety of the children.
- No pets are allowed at any time.
- If attending at Waller, enter through the white gate, safely park your stroller (not in front of the stairs), and find a place to sit.
- Each family brings their own set of instruments (which we provide on the first day that you attend).
- If a class is canceled due to inclement weather, fire smoke, teacher illness, COVID surges, or any reason, you are welcome to schedule a make-up. When possible we will also extend the semester to add an extra class to make up for the canceled day OR we will hold a class on Zoom the day that we cancel.
- If there is a cancellation, we will need to contact you. Please Opt-in to email/text/phone communication here.
- If you miss a class we will offer in-person and on-line make-ups.
- Make-ups are a free courtesy we offer and are not guaranteed within any given semester. Make-ups do not transfer to the next semester unless you register for the next semester.
- Under no circumstances may families drop in to a class unannounced. If you miss class, make-ups must always be scheduled online to ensure that there is no overcrowding.
- Please schedule make-ups. Make-ups require an access code that will be sent out at the beginning of the semester to enrolled families only.
- We regret to inform you that no refunds are issued after the start of a session for any reason at all. The no-refund policy includes COVID surges, natural disasters, weather, and all unforeseen circumstances – all classes will be online if not in-person.
- In-person classes may be canceled or moved online at any time.
BEHAVIOR:
- If your child throws a tantrum or is very loud in a way that is distracting to class please take them outside until they have calmed down. When they have calmed down you are welcome to re-join the circle.
- Please only bring small soft hand-held stuffed animals and small-hand held instruments to keep all of the children safe.
- No throwing or hitting is allowed. If your child is doing so, please intervene immediately: Take them outside until they have calmed down. When they have calmed down you are welcome to re-join the circle as long as the throwing has stopped.
- If they are hitting or throwing, please verbally repeat the "no throwing and hitting in music class" rule to them outside of class AND during class.
- If there is a threat to our safety, we reserve the right to cancel your registration.
COVID-19 ASSUMPTION OF THE RISKS AND WAIVER OF LIABILITY
As consideration for San Francisco Music Together® permitting me to take in-person Music Together classes, I agree to the following:
ASSUMPTION OF THE RISKS: I understand that naturally occurring disease processes (including, but not limited to, the novel coronavirus or COVID-19) may exist in the community, including at San Francisco Music Together, and present an inherent risk that cannot be eliminated. I acknowledge that, although San Francisco Music Together is taking reasonable measures to avoid contact, transmittal, and exposure of disease processes like COVID-19 between people in our classes (including mask enforcement for all adults and children over 2, and strict 6-10 ft social distancing), it is ultimately my responsibility to ensure that I and my child take appropriate actions to safeguard ourselves. I understand and agree that as a registered member of San Francisco Music Together, I am accepting and voluntarily assuming the risk that I or my child may be exposed and become ill as a result of COVID-19 and other diseases.
WAIVER OF LIABILITY: I, on behalf of myself and my child hereby voluntarily release, forever discharge, and covenant not to sue San Francisco Music Together, and its owners, managers, members, employees, agents, and volunteers (“Released Parties”) for any claims that may arise out of or relate in any way to my or my child’s exposure to any communicable disease, including but not limited to the novel coronavirus or COVID-19. The claims hereby released include, but are not limited to, claims of negligence against any of the Released Parties.
I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS CONTENTS. BY ENROLLING IN CLASSES AT SAN FRANCISCO MUSIC TOGETHER, I UNDERSTAND I HAVE GIVEN UP CERTAIN LEGAL RIGHTS AND THAT THIS IS A BINDING LEGAL DOCUMENT.
General Liability Release
I, student/parent/guardian, for and in consideration of the payment to me of music lessons, classes and activities, the receipt and sufficiency of which is hereby acknowledged, do hereby release and forever discharge SF Music Together, their agents, independent contractors, employees, successors and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the following: music lessons, classes and activities, including exposure to contagious disease due to teacher-student proximity or student-to-student proximity. It is understood and agreed that this payment is made and received in full and complete settlement and satisfaction the causes of action, claims and demands mentioned herein; that this Release contains the entire agreement between the parties; and that the terms of this Agreement are contractual and not merely a recital. Furthermore, this Release shall be binding upon the undersigned, and respective heirs, executors, administrators, personal representatives, successors and assigns. This Release shall be subject to and governed by the laws of the State of California. This Release has been read and fully understood by the undersigned and has been explained to me and goes into effect upon the submission of this form.
